The Department of Mental Health, Substance Abuse and Developmental Services is currently seeking a full-time Care Coordinator to provide intensive, community-based case management, care coordination and a facilitated wrap around process to youth and adults with serious emotional disturbance or mental illness. The Care Coordinator will conduct comprehensive psycho-social needs and strengths assessments; develop and implement care plans; facilitate client centered team meetings; provide crisis intervention; facilitate interagency coordination, links and referrals to services and supports; create opportunities to enhance community living skills; provide supportive education and problem solving; and monitor service delivery. The Care Coordinator will work as a member of a multi-disciplinary team to include psychiatrists, therapists, family members and other community service providers. Must maintain all clinical records and documentation according to federal, state and agency policies and procedures. Must be knowledgeable in both mental health and substance abuse assessment; demonstrate crisis intervention skills; possess strong organizational abilities; and demonstrate the ability to function effectively as a clinical team member. Work schedule will include Monday-Friday with evening hours.
Education and experience equivalent to a Bachelor's degree in Social Work, Psychology, or a related field and a minimum of two (2) years directly related experience.
Master's degree preferred; bilingual (Spanish) highly desirable.

Employment is contingent upon criminal background check, driving record check, and TB test. Must have a valid driver's license in Virginia or regional state of residence.
