The San Francisco Department of Public Health (DPH) is currently engaged in a number of transformative initiatives to ensure its integrated delivery system, the San Francisco Health Network (SFHN), continues to meet the challenges of Health Reform, including pay-for-performance programs involving behavioral health integration; implementation of a new electronic medical record (EMRs); integration of Primary Care and Behavioral Health; and full implementation of new reimbursement systems for non-specialty medical health services.

SFHN Primary Care’s vision is for Primary Care Behavioral Health staff to be fully integrated into the patient-centered medical home as members of the primary care team, providing compassionate, effective and efficient care and supporting increased primary care capacity. Based on values of integrity, social justice, and cultural competence, and mindful of the need to revise traditional treatments to incorporate developing technology and to ensure continuing program evaluation, PCBH staff provide comprehensive care of chronic medical and mental illnesses while also embracing the possibility of physical, emotional, social, and spiritual wellness, supporting the health and well-being of patients and the community and the team members providing service.

The Director of Primary Care Behavioral Health (PCBH) reports to the SFHN Primary Care Chief Medical Officer (CMO), with matrix reporting to the Director of Primary Care, and assumes programmatic responsibility for all behavioral health, mental health, and substance abuse services based in Primary Care. The Director of Primary Care Behavioral Health is responsible for the primary care-based integrated behavioral health program. Our fourteen (14) clinics offer a full array of clinical services and all have behavioral health teams as part of the care model. The Director of Primary Care Behavioral Health (PCBH) is a member of the central San Francisco Health Network Primary Care Leadership Team.

Supervising psychologists and social workers in Primary Care report to the Director of PCBH. The Director is the Primary Care lead for cross-organizational collaboration in the areas of behavioral health, mental health, and substance abuse services. These partners include sections within the San Francisco Health Network Zuckerberg San Francisco General Hospital inpatient, specialty, urgent care clinics, and the Department of Psychiatry; the Behavioral Health Services section; Maternal Child and Adolescent Health; Jail Health; Laguna Honda Hospital; Transitions; Population Health; and the Office of Managed Care) as well as outside partners.

The Director of PCBH works closely with the Director of Primary Care Psychiatry to ensure appropriate access to psychiatry services for patients seen in Primary Care. The Director of PCBH meets regularly with the SFHN Director of Behavioral Health Services to ensure successful collaboration between PC and BH, adherence to all regulatory requirements, and programmatic adjustments to meet the changing needs of the population.

ESSENTIAL DUTIES:
The 0931 Manager III –Director of Primary Care Behavioral Health performs the following essential duties:

• Provides the necessary vision and leadership to create and implement a strategy to ensure patient-centered, effective integrated behavioral health care for all SFHN Primary Care patients within their medical homes;
• Chairs the monthly Integrated Continuum of Care committee;
• Participates in and sometimes convenes other San Francisco Health Network meetings and committees related to behavioral health for SFHN Primary Care patients;
• Is accountable for the quality, effectiveness, efficiency, coordination, and patient satisfaction of Primary Care-based behavioral health services;
• Collaborates with other Department of Public Health divisions and SFHN sections on strategies to improve behavioral health, mental health, and substance abuse services;
• Develops appropriate data dashboards and reports of quality improvement and program outcomes;
• Ensures appropriate documentation of all clinical encounters by all behavioral health staff in Primary Care;
• Oversees non-specialty behavioral health billing systems;
• Oversees and participates in hiring PCBH staff;
• Serves as PCBH liaison to DPH supporting departments, such as IT, clinical informatics, budget, and HR;
• Secures additional grant funding to support the development and implementation of new strategies to improve PCBH;
• Supervises and supports PC supervising psychologists and social workers;
• Maintains, promotes and enforces a “customer service” philosophy of care for all PCBH staff;
• Advocates for and supports research and evaluation in the practice setting;
• Plans and implements new PCBH programs in response to new mandates; AND
• Supports PCBH staff in professional development and well-being.

The 0931 Manager III –Director of Primary Care Behavioral Health also performs other related duties as required/assigned.

Compensation and Benefits:
The normal annual salary range is $113,672- $145,106. Appointment above the maximum of the normal range may be considered based on documented and substantiated recruitment and retention issues or exceptional skills. A special approval process is necessary for appointment above the normal salary range.

Minimum Qualifications

1. Five (5) years (10000 equivalent hours) of professional experience as a Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or Licensed Psychologist providing behavioral health services to children and adults. Qualifying experience must include three (3) years (6000 equivalent hours) of experience supervising Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or Licensed Psychologist employees; AND

2. Possession of a valid California license in one (1) of the following disciplines:
• Licensed Clinical Social Worker or Licensed Marriage and Family Therapist issued by the California Board of Behavioral Sciences; OR
• Licensed Psychologist issued by the California Board of Psychology

Desirable Qualifications:

The stated desirable qualifications may be used to identify job finalists at the end of the selection process when candidates are referred for hiring.

• Formal education or training in the areas of health improvement, process improvement, population health, clinical informatics, and/or health systems planning
• Experience in developing, implementing, and enforcing operational procedures
• Strong clinical skills, preferably in primary care settings, including brief evidence-based treatments with a wide range of clinical disorders including chemical dependency
• Excellent computer skills including the utilization of electronic medical records, creating spreadsheets and writing reports
• Experience with patient data collection, patient documentation and healthcare quality management
• Knowledge and previous experience working with regulatory requirements of HIPAA, ADA, Affirmative Action, and the California Department of Managed Healthcare.
• Sensitivity to a multi-cultural, multi-diagnosed, diverse, urban patient population
• Knowledge of management theories/principles; systems and change theories; conflict resolution and customer service skills
• A passion for improving care and for creating a patient-centered healthcare system
• Excellent interpersonal and relationship skills
• Excellent written, verbal, listening, problem-solving, and presentation skills

Selection Procedures

Management Test Battery: (Weight: 100%)

Candidates will be invited to participate in a computer-based examination designed to measure competencies in job-related areas which may include but not be limited to: Problem Solving; Leadership; Decision Making; Interpersonal skill; Human Resources Management; Team Building; Communication; Conflict Management and Process Improvement.  This is a standardized examination and, therefore, test questions and answers are not available for public inspection or review.

 Scores attained on the Management Test Battery will be valid and 'banked' for three years, starting from the date of the examination. This means that, during this three-year time period, you will not be required to take the Management Test Battery. The Management Test Battery may be used for many other classes; therefore your test score may be applied to one or more of these classes if you choose to apply to future recruitments. If the selection process for the future announcement is held within one year of the date of this examination and it includes the Management Test Battery, your score will be automatically applied to that announcement. However, after one year, you have the option to either (a) apply your test score to the other announcement or (b) re-take the Management Test Battery. Re-testing is permitted no sooner than one year from the date of the examination and only in association with your eligibility for another announcement for which the Management Test Battery is used. Please note that, should you re-test, your re-test score would become your official score since it is the most recent.

Certification:
The certification rule for the eligible list resulting from this examination will be Rule of the List.  Additional selection processes may be conducted by the hiring department prior to making final hiring decisions.

Eligible List:
The eligible list resulting from this examination is subject to change after adoption (e.g., as a result of appeals), as directed by the Human Resources Director or the Civil Service Commission.

The duration of the eligible list resulting from this examination process will be 6 months, and may be extended with the approval of the Human Resources Director.

Conviction History

As a finalist for a job, you will be fingerprinted, and your fingerprints will be sent to the California Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI). The resulting report of your conviction history (if any) will be used to determine whether the nature of your conviction (or arrest, in limited circumstances) conflicts with the specific duties and responsibilities of the job for which you are a finalist. If a conflict exists, you will be asked to present any evidence of rehabilitation that may mitigate the conflict, except when federal or state regulations bar employment in specific circumstances, such as:

  • Candidates applying for positions with the Unified School District and the Community College District may be disqualified from consideration should their conviction history not meet the standards established under the California Education Code.
  • Candidates applying for positions with the Recreation and Park Department may be disqualified from consideration should their conviction history not meet the standards established under California Public Resources Code 5164.

Having a conviction history does not automatically preclude you from a job with the City.

Disaster Service Workers

All City and County of San Francisco employees are designated Disaster Service Workers through state and local law (California Government Code Section 3100-3109). Employment with the City requires the affirmation of a loyalty oath to this effect. Employees are required to complete all Disaster Service Worker-related training as assigned, and to return to work as ordered in the event of an emergency.

Conclusion

Licensure/Certification/Registration:
Valid licensure/certification/registration as a requirement to perform the job must be kept current throughout duration of employment. Failure to demonstrate/show proof of possession of required valid licensure/certification/registration may result in dismissal and/or termination of employment.


Note on Personal Protective Equipment (PPE):
Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing.

Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a FaceFitting Respirator.

Medical Examination:
Prior to appointment, at the Department’s expense, applicants may be required to take a tuberculosis (TB) screening test, medical examination, and/or drug test.
