Camp Mather 2017 Season 3264 Camp Assistant Supplemental Questionnaire
This Supplemental Questionnaire gathers job-specific information in order to expedite the selection process and attend to some administrative matters. Please read all of the items carefully and respond to each completely.
CERTIFICATION - I self-certify that I am a high school graduate, OR that I have received my GED, OR that I will have graduated from high school by 30 June 2017. You MUST attach to this application documentation which validates your high school status. Documentation, in PDF format, may comprise a high school diploma; OR a letter from your high school, signed by an official representative, which states that you will have graduated by 30 June 2017; OR a copy of your GED; OR a transcript from your most recent semester/quarter of college attended.

Camp Assistants work 6-days per week, as needed, and staff a variety of shifts, including weekends. The 2017 season begins on April 1 and ends on September 23. The season is divided into three phases: startup, high season, and wind-down. Please select which, if any, of the following phases you are available to work without interruption:
I can work from April 1 thru May 27 without an interruption in service (startup)
I can work from May 28 thru August 12 without an interruption in service (high season)
I can work from August 13 thru September 23 without an interruption in service (wind-down)
I cannot work, without interruption, during the full term of any of the above phases

Camp Mather is a family venue with guests of all ages. We need well-rounded staff who can work with guests, enhance their camp experience, and help them to enjoy a wonderful vacation. There is a full recreation program 7 days per week. The next 5 items gather information to help determine whether your interests align with Camp Mather's recreational offerings.

We present a staff talent show for guests. Participation in this talent show is mandatory. In the box below, please provide examples of how you envision yourself playing a role in our talent show.

Four hunderd (400) hours of experience as a paid or volunteer lifeguard
Lifeguard training certification issued by the American Red Cross, YMCA or Ellis and Associates
ServSafe certificate or California Food Handler Card
Waterfront Lifeguard certification
