The purpose of this Supplemental Questionnaire is to determine your knowledge, skills, and abilities in job-related areas that have been identified as critical for satisfactory performance in this position. All relevant experience, education and/or training must be on the supplemental questionnaire in order to be reviewed in the minimum qualifications and rating process to determine candidates’ score on the Supplemental Questionnaire Evaluation. This information should be consistent with the information on your application (employment record, not resume) and is subject to verification.

All of your responses to the questionnaire MUST be supplied in the spaces provided. Please provide all information requested even if the information may appear redundant. Please be thorough but concise. Attachments or additional documents such as resumes, cover letters, or application will NOT be considered. (i.e. Writing “see resume/website/application” is not sufficient response.). Failure to do so may result in disqualification or a lower score on the examination.

There are three (3) questions that you must answer by giving specific examples of your most relevant experiences. Describing a variety of experiences will provide the evaluating committee with a better understanding of the breadth and depth of your experiences. Include the name of the employer(s), dates of experience, and the name and phone number of verifiable sources for EACH question.

1. Describe your experience analyzing and monitoring capital projects with multiple funding sources, including grants that created multiple types of capital assets. Indicate in your response:

a. the name , project budget, and scope of work of the capital projects you worked on.
b. the different types assets created, how the asset types are identified and segregated using GASB pronouncements
c. the funding eligibility rules applied to ensure the compliance with the restricted funding sources d. the name and types of grants, the eligibility rules required by the grant and the criteria you used to comply with the grant requirements.

2. Describe your most challenging experience in identifying and resolving issues with a federal/capital grant reimbursement package using a specific project. Indicate in your response:

a. The name of the project you worked on.
b. The individuals and/or agencies that you have dealt with
c. Your role, its outcome, and why the experience was challenging
d. actions required to resolve the issues

3. Describe your experience with financial audits, such as the single audit, post audit, financial statements, and compliance audits. Include in your response the following:

a. Describe in detail your role in the audit process, ideally a single audit and development of financial statements.
b. Outline on an audit schedule you have been involved in, which sources and uses are reported and what were the adjusting or reconciling items?
