Main Purpose of Job
The Nurse Case Manager will assist the Program Manager in capacity development activities for development of a syringe services/harm reduction program within the Washington County Health Department. These activities will include developing a referral resource inventory, identifying and establishing collaborations with local partner agencies, conducting key informant and public input groups to inform SSP proposal, gathering community data, making presentations to local elected officials and conducting outreach to community stakeholders to build support for syringe services and harm reduction strategies, and developing an application for Syringe Services Program implementation for submission to DHMH. Contingent on acceptance and funding of the application for syringe services program implementation, this position will coordinate the communicable disease activities of a syringe services program which includes the coordination of HIV, hepatitis, STD testing and referrals for treatment, referrals for PREP, vaccination outreach, referrals for insurance navigation, referrals for primary care, wound care or other medical care.
Education: A Bachelor’s degree in nursing from an accredited college or university.
1. Candidates may substitute a Bachelor’s degree in a related field from an accredited college or university and one year of experience as a registered nurse for the required education.
2. Candidates may substitute two years of experience as a registered nurse for the required education.
3. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Nursing classification or Nursing or Health Care Specialist specialty codes in the Nursing field of work on a year-for-year basis for the required education.
LICENSES, REGISTRATIONS AND CERTIFICATIONS
1. Candidates for positions in this classification must possess a current license as a Registered Nurse from the Maryland State Board of Nursing, 4140 Patterson Avenue, Baltimore, Maryland 21215. A copy of your license must be attached to your application.
2. Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator’s license valid in the State of Maryland.
Employees in this classification will be required to provide the facility with a telephone number at which they can be reached.

Contractual employees who works for an agency covered under the State Employee and Retiree Health and Welfare Benefits Program (the Program), have a current employment contract and work 30 or more hours a week (or on average 130 hours per month) may be eligible for subsidized health benefits coverage for themselves and their dependents. As a contractual employee, you will be responsible for paying 25% of the premiums for your medical and prescription coverage, including any eligible dependents you have enrolled. The State of Maryland will subsidize the remaining 75% of the cost for these benefits. You can also elect to enroll in dental coverage, accidental death and dismemberment insurance, and life insurance, but will be responsible to pay the full premium for these benefits.
