Main Purpose of Job

This position is responsible for the oversight and management of the Coordination of Community Services (CCS) funded through the federal Targeted Case Management model. CCS services are provided to approximately 16,000 individuals in DDA funded services and to approximately 8,000 people on the Waiting List. Services are provided by 21 private and public entities. Coordination of Community Services is the foundation of all DDA programs. Coordinators work with individuals to conduct eligibility assessments, assess needs, identify service providers and natural supports, assist with waiver and other program applications, and monitor services to ensure they are being delivered as specified in the individual's plan. Coordinators also play a role in monitoring the health and safety of individuals. The purpose of this position is to oversee all aspects of the CCS program and act as a representative of DDA in discussions with coordination providers, individuals, families, and other stakeholders. The successful applicant should have experience in working with individuals with intellectual and developmental disabilities. The successful applicant should have training and experience in providing case management services and project management, be able to analyze data, and be able to develop and conduct training related to case management services for service providers.

MINIMUM QUALIFICATIONS

Education: Possession of a bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling, or a related field.

Experience: Six years professional experience in health services, three of which must have been in professional work related to treatment and development of developmentally disabled clients, and two years of which must have been at the managerial or supervisory level.

Notes:

1. Applicants may substitute a master’s or Doctoral degree in a health or human services field for up to two years of the required general experience.

2. A bachelor’s degree from an accredited college or university in another field plus an additional year of professional experience in health services may be substituted for the specific degree.

3. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Health Services Administration specialty codes in the health related field of work on a year-for-year basis for the required experience and education.
