JOB OVERVIEW

Performs duties collecting and analyzing socio-economic information, and arranging for comprehensive plans to deliver social service programs and promote self sufficiency of eligible clients.

MINIMUM QUALIFICATIONS

An Associate's degree; and

Two years of experience assessing client eligibility for social service programs; and 

Possession of a valid Florida Driver's License; and

The ability to speak both English and Spanish.

OR

An equivalent combination of education, training and experience that would reasonably be expected to provide the job-specific competencies noted below; and

Possession of a valid Florida Driver's License; and

The ability to speak both English and Spanish.

PHYSICAL DEMANDS AND WORK ENVIRONMENT

Physical Demands:

  • Position requires ability to lift/carry, bend/crouch, push/pull, squat/kneel, reach, data entry, walk, sit, stand, and file.
  • Ability to assist with manuevering a wheelchair or assisting seniors with limited mobility.
  • Ability to multi task and use a computer and telephone answering system.

Work Environment:

  • Position will primarily work out of an assigned Adult Day Care Center.  Significant time will be spent conducting client assessments.
  • Travel will be required for marketing, training, staffing at another Adult Day Care center, special events and meetings. 
  • Work environment requires candidate to multi-task, the ability to work in an area with noise in the background and may assist to redirect confused seniors throughout the day.
  • Performs duties collecting and analyzing socio-economic information, and arranging for comprehensive plans to deliver social service programs and promote self sufficiency of eligible clients.

JOB SPECIFIC COMPETENCIES

  • Working knowledge of the theories, techniques and methods of social services program delivery, and case management procedures.
  • Working knowledge of Federal, State, County, and community social service programs for which most clients are eligible: eligibility requirements, application procedures, and other relevant program policies.
  • Skill in interviewing clients to identify deficiencies in personal, legal, social, economic, and health needs.
  • Skill in counseling others in attaining self sufficiency.
  • Skill in crisis intervention techniques.
  • Skill in dealing with social service clients under stressful conditions.
  • Ability to work effectively with others.
  • Ability to collect, organize and evaluate data and to develop logical conclusions.
  • Ability to correctly apply applicable laws, regulations, and policies to individual cases.
  • Ability to use a computer and related software.
  • Ability to communicate effectively, both orally and in writing.

REPRESENTATIVE DUTIES

  • Analyzes a comprehensive body of social, economic, legal, environmental, occupational, physical and mental health information derived from in depth client interviews, observation, research and investigation; to assess client needs, to determine client eligibility for services, to determine causes of client's situation, to develop recommended services and courses of action to satisfy client needs, and to develop a case management plan to be used to evaluate service delivery, follow up, reassessment and service modification which will result in client self sufficiency in any one of the programs sponsored by aging services, children's services, health and social services, head start, the correction system, or the court system.
  • Counsels individuals, families and groups; may perform crisis intervention; advocates for clients; and collaborates with community service agencies to provide information and services necessary to meet various socioeconomic needs in a variety of settings, to improve situations and restore to self sufficiency in a community setting.
  • Writes comprehensive client social histories; social services program procedures; case management plans, reports, studies and summaries; and other pertinent information used to document contacts, resources used, performance of service providers, satisfaction of client needs in conformance with local, State and Federal guidelines, and to improve and enhance program delivery.
  • May conduct surveys, research references, and develop programs; and plans, coordinates, facilitates, conducts, and evaluates training, workshops and other activities designed to enhance client abilities to eliminate their need for support services and become self sufficient.
  • Provides client assessment and other related information to public officials and others for their use in determining appropriate courses of action relative to client needs and situations; coordinates with social service agencies, psychologists, law enforcement, attorneys and judges to develop requirements to satisfy client needs; and attends or conducts meetings to exchange social service information.
  • Compiles data for use in evaluating program performance, reports, service provider payments, program documents, financial needs, and budgeting, using manual methods and personal computer applications.
  • Performs other related duties as required.

  • Health Plan: The County pays towards health insurance premiums for all employees. Employees can select from three health care plan options. Coverage is effective the first of the month following 60 days of employment.
  • Dental Insurance: Employees are eligible to enroll in the dental coverage and choose from four options: High DMO, Low DMO, a network Open Access Plan, and a PPO. Coverage is effective the first of the month following 30 days of employment.
  • Vision Care: Employees and their dependents are eligible to enroll in the vision coverage. Co-payment coverage includes exams, lens and frame, as well as an allowable contact lens expense. Coverage is effective the first of the month following 30 days of employment.
  • Employee Assistance Program: This program provides all employees and family members with professional, confidential counseling sessions. All employees and dependents are afforded up to six (6) free visits per Plan year. Coverage begins on the first day of employment.
  • Tuition Reimbursement: Employees may be reimbursed up to $1,000 (undergraduate), $2000 (graduate) per fiscal year for classes at an accredited college or university for courses that enhance your ability to perform your current job duties.
  • Holidays: Employees receive up to 12 paid holidays per year.
