DESCRIPTION

HEALTH CARE SERVICES AGENCY
Alameda County's Health Services Program is administered by the Health Care Services Agency and includes the following program areas: Behavioral Health Care, Public Health, Environmental Health, and the Office of the Agency Director. The ultimate mission of the Health Care Services Agency is to provide fully integrated health care services through a comprehensive network of public and private partnerships that ensure optimal health and well-being and respect the diversity of all residents. 

CENTER FOR HEALTHY SCHOOLS AND COMMUNITIES
The Center for Healthy Schools and Communities (CHSC) is part of the Alameda County HCSA and provides services to school age youth from pre-kindergarten through community college. The CHSC represents many diverse collaborations of service providers, school and school district leaders, health advocates, community partners, policy makers, and youth working to create equity in education and health for all students. The CHSC fosters the academic success, health and well being of Alameda County youth by building universal access to high quality supports and opportunities in schools and neighborhoods.  We value empowering families and youth, growing the capacity of communities to affect change and building strategic partnerships that link health and education institutions to achieve equity.  We envision a county where all youth graduate from high school healthy and ready for college and career.

THE POSITION
Clinical Case Managers, Center for Health Schools and Communities (CHSC) positions are located in the Health Care Services Agency, Center for Healthy Schools and Communities and report to a Behavioral Health Care Services Manager, or a School Health Services Administrator, CHSC (only licensed staff will provide clinical supervision). Clinical Case Managers, CHSC spend the majority of their time at a school, district or community-based site where they are assigned and work in close collaboration with leadership, support staff, and service providers. Incumbents support the CHSC by providing and/or coordinating the following:

  • Mental health consultation and psycho-education with teachers, staff and parents;
  • Leadership, social skills and counseling groups with students;
  • Time limited individual therapy with students;
  • Case management services such as parent/caregiver support, outreach and education, home visitation, screening and assessment of psychosocial needs, crisis response and intervention, and other appropriate adjunct clinical/therapeutic services.

Clinical Case Managers, CHSC also coordinate or participate in site efforts to create a positive climate and learning environment; promote psychosocial wellness and decrease student exposure to harm; increase student resiliency; and strengthen other systems of support for youth and families. Incumbents may serve as the CHSC primary contact at the
site(s) in which they work and may convene other program staff and partner agencies on a regular basis to coordinate activities and strengthen delivery of all CHSC components.

As a member of the Center for Healthy Schools and Communities (CHSC) staff, the Clinical Case Manager, School Based will be contributing to executing the following core CHSC strategies:

  1. Supporting the development and sustainability of a Community Schools and Neighborhood (CSN) strategy to promote equity.
  2. Creating universal access to health and learning [supports] and opportunities in schools and neighborhoods.
  3. Creating safe, healthy, culturally-responsive school and neighborhood environments where everyone thrives.
  4. Strengthening culturally-responsive, quality practices for children, youth and families with a focus on those furthest away from opportunity. 
  5. Strengthening partnerships between schools and youth and their families to promote academic success and community health and wellness, healing, capacity and action.
  6. Aligning cross-sector systems, policies, and practices around a collective impact approach to advance our equity goals.

THE IDEAL CANDIDATE
In addition to meeting the minimum qualifications, the ideal candidate will be a dynamic case manager with solid experience managing the needs of youth and coordinating efforts with parents and teachers as well as school and health care administrators. The ideal candidate will have demonstrated expertise and experience in the following areas:

  • Applying theories and principles of mental health consultation, multidisciplinary teams, group and individual therapy/counseling, family support, trauma-informed practice, cultural responsiveness, positive school/community environments, and systems-level interventions;
  • Utilizing the principles and practices of behavioral health supports in the unique settings of schools and community centers;
  • Conducting culturally responsive assessments of psychosocial needs of youth and their families and a track record of strengthening internal and external systems for student referrals;
  • Identifying available youth and family resources, with the knowledge and ability to assess eligibility for a variety of health care and social service programs;
  • Strengthening and supporting school-wide efforts to create a positive climate, prevent bullying and violence, and enhance the school community for all of its members; 
  • Providing quality mental health services in a school or youth center setting to support youth with  experiencing disruptions to their academic engagement including, but not limited to exposure to violence and trauma, mental health issues and lack of basic needs;
  • Participating on site-based multidisciplinary teams who work together to address support needs of referred children/youth and manage support resources in the school as well as providing consultation to other multidisciplinary team members and site staff;
  • Managing, tracking and reporting data on the status of student progress and systems development;
  • Managing a caseload while staying focused on a variety of critical priorities, including systemic mental health prevention efforts, consultation and psychoeducation;
  • Aligning program service objectives with individual performance goals;
  • In addition, bi-lingual skills, school-based experience, a strong background in both school and community settings and eligibility for a Board of Behavioral Sciences license are desired.

 

MINIMUM QUALIFICATIONS

Education:

Possession of a Master’s degree from an accredited college or university in social work, marriage and family therapy, clinical or educational psychology or in a closely related field.

AND

Experience:

The equivalent of two (2) years full-time post graduate case management experience with youth (5-24 years of age) and families in a school or community setting that serves children, youth, and families and are linked to educational outcomes.

(Possession of a Pupil Personnel Services Credential (PPSC) may be substituted for 18 months of the required experience.)

AND

License:

Possession of a valid California Driver’s license is required.

KNOWLEDGE AND SKILLS

The most suitably qualified candidates will possess the following competencies:

Knowledge of:

• Developmental, academic, social/emotional and mental health needs of school-age children and youth (5-24 years of age).
• Theory and principles of mental health consultation, multidisciplinary teams, group and individual therapy/counseling, family support, trauma-informed practice, cultural responsiveness, and positive school/community environments.
• Social justice and systems change theory; and personal and collective healing and transformation.
• Youth culture and needs; and principles of Positive Youth Development.
• Socio-cultural factors impacting children, their families, and the community.
• Culturally competent, bio-psychosocial assessments of children/youth, their families, and their social and school environments.
• Screening, intake, and assessment procedures.
• Basic methods of clinical intervention such as counseling, crisis intervention, casework, group work, advocacy, consultation, case management, family therapy, and strength/asset-based approaches.
• Conflict resolution, violence prevention, and substance abuse treatment and prevention.
• Principles and practices of behavioral health supports in the unique settings of schools and community centers.
• Principles and practices to facilitate linkages to community agencies and community resources and their role in prevention and treatment of at-risk children and youth.
• Legal and ethical guidelines governing requirements for reporting sexual and physical abuse and other exceptions to rules of confidentiality.
• Applicable federal, state and local laws, rules and regulations.
• Basic health insurance systems and operations related to the enrollment process.
• Computer applications related to the work.

Ability to:
• Develop positive, supportive professional relationships with a variety of individuals and groups that are part of a school or YF Center community such as teachers, staff, students, families, and service providers.
• Manage a site based school based behavioral health service plan to ensure that a full continuum of services including prevention, early intervention, and intensive treatment are delivered.
• Build effective partnerships between school personnel, county children and youth systems and community-based providers who work together towards achieving common goals.
• Serve as an effective resource at the assigned site(s), while also publicly representing the core mission, strategic directions, and initiatives of the CHSC.
• Use culturally relevant outreach strategies for developing positive relationships with parents and families, engaging them in services that support their children, and providing family support as needed.
• Learn and use holistic healing modalities.
• Determine mandated parental consent requirements for the treatment of minors.
• Work collaboratively with multi-disciplinary teams, community resources and organizations.
• Understand confidentiality in school settings and establish and maintain confidentiality as appropriate and mandated.
• Work in diverse community settings with families from diverse cultural, ethnic, linguistic and economic backgrounds.
• Manage multiple and changing priorities.
• Interpret, explain and apply relevant regulations, policies and procedures.
• Communicate effectively, orally and in writing.
• Prepare clear, accurate and effective reports, correspondence and other written materials.
• Establish and maintain effective working relationships with those contacted in the course of work including administration, school or YF Center community members, and all CHSC staff and management.
• Demonstrate ability to work with various at-risk populations such as youth not attending school, youth in out-of-home placement, homeless youth, delinquent youth, and youth abusing alcohol or other drugs.
• Understand the critical aspects of conducting home visitations, such as home assessment, triage, strategies for personal safety, and cultural competence with special consideration for disability, race, and poverty.

EXAMINATION COMPONENTS

The examination will consist of the following steps: A review of candidates' applications to verify possession of minimum requirements. Those candidates who possess the minimum requirements for the class will move on to the next step in the examination process which is a review of candidates' Supplemental Questionnaires to select the best qualified applicants to continue in the process. Those candidates selected as best qualified will move on to the next step in the examination process, an oral interview which will be weighted as 100% of the candidate's final examination score. The oral interview may contain situational exercises. CANDIDATES MUST ATTAIN A QUALIFYING RATING ON EACH PORTION OF THIS EXAMINATION. We reserve the right to make changes to the announced examination components.
