Medical Records Technician

Introduction

THIS IS A NEW ASSEMBLED EXAMINATION.  The eligible list resulting from this examination will cancel any existing list and may last approximately one year, but can be extended. Applications must be in the possession of the Human Resource Services Department by 5:00 p.m. on the Last Day for Filing.  Postmarks are not accepted.  Applications will only be accepted on-line.

Supplemental Questionnaire:  A properly completed Supplemental Questionnaire must be submitted with each application.  Applications and Supplemental Questionnaires must be in the possession of the Human Resource Services Department by 5:00 p.m. on the Last Day for Filing. Postmarks are not accepted.  Failure to submit the Supplemental Questionnaire will result in disqualification.

Qualified Bilingual Persons are Encouraged to Apply:   

Bilingual persons who speak English and are also fluent in Spanish, Chinese, Vietnamese, Cambodian, Laotian, Korean, Mien, Tagalog, Amharic, Farsi, Dari, Tigrigna, Russian, Romanian, or Sign Language are especially encouraged to apply.  There is an additional bi-weekly compensation for persons in positions designated bilingual. DESCRIPTION

THE HEALTH CARE SERVICES AGENCY

Alameda County's Health Services Program is administered by the Health Care Services Agency and includes the following program areas: Behavioral Health Care, Public Health, Environmental Health, and Agency Administration/Indigent Health. The ultimate mission of the Health Care Services Agency is to provide fully integrated health care services through a comprehensive network of public and private partnerships that ensure optimal health and well-being and respect the diversity of all residents. PUBLIC HEALTH DEPARTMENT (PHD)

CALIFORNIA CHILDREN’S SERVICES (CCS) MISSION
California Children’s Services of Alameda County promotes optimal health outcomes for children and youth with special health care needs through compassionate, comprehensive, and specialized collaboration with families and health care providers.  CCS provides medical case management and care coordination to low-income children under 21 years of age with chronic and disabling medical conditions. CCS can help arrange and pay for hospital stays and doctor visits, medical equipment, medication, and other services. CCS strives to ensure that children receive appropriate and comprehensive care.

THE POSITION

Under direction, to code and abstract diagnoses and procedures according to the latest adaptation of the International Classification of Diseases Coding (ICD-9 CM or ICD-10CM) classification system; to compile statistical indices and data sources used for medical audits and other statistical reports; and to perform related duties as required.   Positions in this para-professional classification are located within the Health Care Services Agency’s Public Health Department’s, California Children’s Services Program. Incumbents in this class receive supervision from a Medical Records Manager.  

THE IDEAL CANDIDATE

In addition to meeting the minimum qualifications stated below, the ideal candidate will have demonstrated a track record of success in the following areas:

  • Requesting medical reports and compiling medical records data for chart content and required documentation;
  • Analyzing medical information to determine appropriate medical codes according to the latest adaptation of the International Classification of Diseases Coding System (ICD-10CM);
  • Completing service authorization requests;
  • Reviewing medical record reports for clarity and accuracy;
  • Consulting with medical staff as needed;
  • Contacting providers for service request clarification; and
  • Compiling statistical indices and data sources used for medical audits and other statistical reports.

MINIMUM QUALIFICATIONS

- Education:

Successful completion of coursework in medical terminology, human anatomy and physiology.

AND

- Experience Pattern I:

The equivalent of one-year recent, within the last five (5) years, full-time experience as a Public Health Nursing Assistant, or in an equivalent or higher level class, that included work with medical terminology and knowledge of medical record content in the Alameda County classified service (non-classified includes District Attorney’s Office, Hospital Authority, and the Consolidated Courts).

OR

- Experience Pattern II:

The equivalent of one-year recent, within the last five (5) years, full-time medical clerical experience that required using and interpreting a wide variety of medical terminology in ICD-9CM or ICD-10CM coding, in a physician’s office, hospital, medical clinic or a medical billing setting.

 

NOTE: The Civil Service Commission may modify the above Minimum Qualifications in the announcement of an examination.

KNOWLEDGE AND SKILLS

The most suitably qualified candidates will possess the following competencies:

Knowledge of:

• Current adaptation of the International Classification of Diseases coding system used for coding and classifying diseases, diagnoses and medical procedures (i.e., ICD-9CM or ICD-10CM).
• Components and format of medical records.
• Medical terminology
• Basic human anatomy and physiology.
• Statistical techniques used for compiling, displaying and analyzing data.
• Current HIPPA regulations regarding privacy, security and confidentiality as it pertains to Protected Health Information (PHI).
• Microsoft Word, Outlook, Excel and data entry programs.

Ability to:

• Read and comprehend the technical elements of a medical record.
• Analyze and code medical records at a 95% accuracy level.
• Recognize and follow up inconsistencies in medical records.
• Prepare clear and concise narrative, statistical, and graphic reports.
• Communicate clearly and professionally with staff, families and providers.
• Communicate clearly and effectively in English both verbally and in writing.
• Provide and promote excellence in customer service for both internal and external customers.
• Work on a computer database system; learn to load software and back up data.
• Set work priorities and work independently; exercise considerable independent judgment.
• Lift, move and/or carry up to ten (10) lbs., as well as working sitting or standing for extended periods of time.

EXAMINATION COMPONENTS

THE EXAMINATION WILL CONSIST OF the following steps:

1) A review of candidates' applications to verify possession of minimum requirements. Those candidates who possess the minimum requirements for the class will move on to the next step in the examination process.

2) A review of candidates’ Supplemental Questionnaires to select the best qualified applicants to continue in the process.

3) An oral interview which will be weighted as 100% of the candidate's final examination score. The oral interview may contain situational exercises.
