Applicants for this position are required to submit responses to the following supplemental questions.  Your completed responses to the supplemental questionnaire will be evaluated to determine your qualifications and must be completed properly in order to be given full consideration for the next phase in the selection process. Additionally, your responses will also be evaluated and used in the selection process in order to identify the best qualified applicants.

Please describe your supervisory and administrative experience which includes program planning, development and evaluation, fiscal, and personnel management in a community organization or public agency related to maternal and child health.

Describe your experience supervising, training, reviewing and evaluating the work of a multidisciplinary, service delivery staff, accentuating experience that pertains to maternal, child and adolescent health programs.

Describe a recent example of a recommendation you made to implement a maternal, child, and adolescent health program policy. Please describe how you developed your recommendation and describe your role in initiating a plan of action for implementing the policy to a diverse group of stakeholders. What was the result?

Describe an experience where you were responsible for performing highly complex data analysis, research, and preparing complex reports to support policy, budget, or program management decision making.  What methodologies did you use?  What were your findings?  How was the information used, and what was the end result?
