Introduction

Help to improve access to care and resources for the homeless in Solano County.

The Department of Health and Social Services is recruiting for two new positions:

Community Services Coordinator—Mentally Ill Homeless Outreach 

The Community Services Coordinator position, will work in the Housing & Community Services Unit in the Health and Social Services Department - Administration Division.  This position will be charged with coordination of County and allied community programs directed at serving homeless mentally ill individuals.  This position will work closely with the homeless mentally ill outreach team (to include contract providers) to ensure mental health outreach, assessment and linkage activities while assuring a coherent approach to homeless outreach for the mentally ill throughout the County.  This position will coordinate a multi-disciplinary approach to meet the needs of the homeless mentally ill, coordinate activities with the homeless mentally ill outreach team and the related activities of homeless outreach workers within the County, and others, while also being responsible to respond to community requests for assistance, planning and program consultation where issues of serious mental illness are concerned.   

Community Services Coordinator—Black Infant Health Program 

The Black Infant Health Program (BIH) is recruiting for a Community Services Coordinator to ensure that the program is implemented according to State standards.  The Community Services Coordinator reports to the Maternal, Child and Adolescent Health (MCAH) Manager and supports the program by implementing the BIH program in accordance with established Black Infant Health policies and procedures in order to provide primary support and appropriate oversight to the staff, and to build and model a strengths-based, client-centered culture. The Coordinator must possess knowledge about community organizations and community resources serving the African American community and demonstrate an understanding of the complex, interrelated issues and concerns contributing to health disparities affecting African Americans. In addition, the Coordinator will be responsible for conducting chart audits, data analysis, and other quality assurance functions to maintain program adherence. 

The Position

Under general direction of a higher level Manager, Administrator or Director, plans, organizes, coordinates, and oversees one or more programs that address a continuum of care including community support, social rehabilitation, health care, HUD funded/sponsored housing assistance, and/or behavioral health services for clients and their families; participates in the development, recommendation, implementation and evaluation of goals, objectives, policies and procedures related to program area; coordinates and supports local agencies in providing assistance and services; participates in or acts as a liaison with policy and planning groups; participates in contract development and monitoring, quality assurance activities, and in developing and monitoring the assigned budget. Provides staff support to higher level management. All work is performed in accordance with professional standards.

The eligible list established from this recruitment will be used to fill part-time and full-time regular, limited-term or extra-help positions as vacancies occur or the need arises.

POSITION REQUIREMENTS:

Education: A Bachelor's degree or higher from an accredited college or university with a major in Psychology, Social Work, Public Administration, Health Services Administration, or a closely related field. A Master's degree in a related field is desirable

AND

Experience: Three years of full-time paid experience in a public sector, health, mental health, social services, housing, or community-based services related program of which one year was in a supervisory, program coordinator or lead capacity.  

LICENSING, CERTIFICATION AND REGISTRATION REQUIREMENTS:  

Applicants are required to possess a valid California Driver’s License, Class C.  Note:  The driver’s license must be kept current while employed in this class.

BENEFITS

Solano County offers a comprehensive benefit package to full and part-time employees which includes a cafeteria-style medical package with health benefits offered through CalPERS. The County contributes 75% of the Kaiser family rate for the Bay area for coverage that begins the first of the month following date of hire. The County offers a cash back provision for those who choose employee-only medical coverage or those who waive medical insurance coverage. Dental and vision insurances for the employee and eligible dependents are paid 100% by the County.

Solano County participates in CalPERS retirement, and contributes to Social Security.

Vacation is accrued at approximately 10 days per year for the first 78 full time pay periods (approximately 3 years). Sick leave accrues at approximately 12 days per year. Additionally, the County observes 12 fixed, paid holidays per year.

The bargaining agreement for this position expires on September 28, 2015. The union is currently in bargaining with the County for wage, hours, benefits and other working conditions for employment.

Selection Process

Veterans Preference Points

To be eligible, applicant must have served at least 181 consecutive days of active duty in the Armed Forces of the United States and have received either an honorable discharge or a general discharge under honorable conditions.  A copy of the DD214, showing discharge type, must be received in the Human Resources Department by the final filing date.
