CLASS 2303 PATIENT CARE ASSISTANT
APPOINTMENT TYPE:

Under direct supervision of a licensed nurse, the Patient Care Assistant performs essential job functions which include:
· Provides routine patient care in accordance with established policies, procedures and standards;
· Establishes and maintains a therapeutic and/or medical relationship with patients;
· As appropriate, reviews patient's admission documentation and notes pertinent facts regarding diagnoses and treatment history. Documents care according to department standards;
· Maintains constant and continuous communication with medical providers and staff;
· As directed by licensed staff; reviews and implements treatment plans, and provides one-to-one care when essential to treatment plans;
· Observes, records and reports patient medical, social and psychiatric behavior and physical conditions;
· Participates in interdisciplinary team meetings and treatment planning by reporting observations and concerns and by asking questions;
· Attends and participates in patient groups, case conferences, staff meetings and in-service training programs;
· Performs other related duties as assigned/required.
MINIMUM QUALIFICATIONS:
1. Six (6) months of verifiable experience as a health care worker within the last five (5) years working in a health care setting; AND
2. Possession of a current Certified Nursing Assistant certificate.
NOTES:
1. San Francisco General Hospital positions also require the possession of a valid Cardiopulmonary Resuscitation (CPR) certificate issued by the American Heart Association. Applicants who do not possess such a certification may participate in this recruitment process and be appointed to a position(s), but will be required to obtain the certificate within two (2) weeks of hire date and maintain validity throughout length of employment. A photocopy of the valid Cardiopulmonary Resuscitation (CPR) Certificate must be provided to the appropriate supervisor of the appointment within the allotted time frame. Failure to demonstrate/show proof of possession of valid certification within two (2) weeks of hire date and throughout length of employment may result in dismissal and/or termination of employment.
2. Some positions require the possession of a Certified Phlebotomy Technician I (CPT-1) certificate, issued by the State of California Department of Public Health.

Computer kiosks are located in the lobby of the Department of Human Resources, 1 South Van Ness Avenue, 4th Floor, San Francisco, for use by the public. The hours of operation are from 8:00 a.m. to 5:00 p.m. Monday through Friday.
VERIFICATION OF EXPERIENCE/CERTIFICATE:
Applicants may be required to submit verification of qualifying experience and proof of certifications at any point in the application, and/or departmental selection process.
SELECTION PROCEDURES:
The selection process will include evaluation of applications in relation to minimum requirements. Depending on the number of applicants, the Department may establish and implement additional screening mechanisms to comparatively evaluate qualifications of candidates. If this becomes necessary, only those applicants whose qualifications most closely meet the needs of the Department will be invited for an interview.
NOTES:
- Prior to appointment, at the Department’s expense, applicants may be required to take a TB screening test, and a medical examination.
