2233 SUPERVISING PHYSICIAN SPECIALIST

Under general administrative direction, the Supervising Physician Specialist plans, organizes, directs and is responsible for the overall operation of a specialty clinic, center program, other patient care site or a professional group. The essential functions include: directing a specialty clinic center program or other patient care site; planning, directing and evaluating medical and/or other professional staff development; conducting meetings and conferences with other agencies and representatives; providing and/or supervising direct medical treatment; evaluating community needs; and directing quality management activities. The Supervising Physician Specialist also performs related duties as required.
MINIMUM QUALIFICATIONS: (See “Verifications” section below)
1a. Possession of a Doctor of Medicine (M.D.) Degree and license to practice medicine issued by the Medical Board of California, 2005 Evergreen Street, Suite 1200, Sacramento, CA 95815; (916) 263-2382; OR
1b. Possession of a Doctor of Osteopathy (D.O.) Degree and license to practice medicine issued by the Osteopathic Medical Board, 1300 National Drive, Suite #150, Sacramento, CA 95834-1991; (916) 928-8390; AND
2a. Three (3) years of post-residency experience in Primary Care; family medicine, internal medicine or pediatrics, including two (2) years of supervisory experience.
CONDITIONS OF EMPLOYMENT:
All qualified candidates who have been selected for appointment to positions in billable specialty areas must be an eligible billable provider and where applicable will be required to meet the following criteria:
· Be eligible to participate in Medicare, Medicaid and/or other federal health care programs;
· Possess a National Provider Identifier (NPI);
· Submit a completed credentialing application and/or required documentation for credentialing; AND
· Possess a valid third-party billable provider certification (such as Medicare, Medi-Cal and/or private insurance) OR have submitted a completed billable provider application, along with the required documentation, in order to obtain the appropriate billable provider status.
Failure to meet these criteria and/or ‘Conditions of Employment’ where applicable within two (2) weeks PRIOR to start work date may result in the delay of appointment and/or cancellation of employment offer. Once appointed, all qualified candidates/employees will be required to retain these qualifications throughout length of employment. Failure to demonstrate (show proof) of qualifications shall result in the termination of employment.

Computer kiosks are located in the lobby of the Department of Human Resources, 1 South Van Ness, 4th floor, San Francisco, CA 94103 for use by the public. The hours of operation are from 8:00 a.m. to 5:00 p.m. Monday through Friday.
VERIFICATIONS:
Verification of education, licensure and board certification/eligibility: Verification of education, licensure and board certification/eligibility must be submitted at the time of filing. Applicants must submit: a photocopy of their degree and/or transcript from an accredited school of medicine; a copy of their license to practice medicine; and a copy of their certification or registration to a specialty board as issued by the American Board of Medical Specialties as proof of qualifying education, licensure and board certification/eligibility.
Verification of experience: Verification of all experience needed to qualify must be on the employer’s letterhead indicating the name of the applicant, job title, dates of employment, hours per week, duties performed and must be signed by the supervisor and/or appropriate representative. City employees do not need to submit verification of qualifying City work experience if experience gained is from the class to which appointed. Credit for experience obtained outside of the employee’s class will only be allowed if recorded in accordance with the provisions of Civil Service Commission Rules. City employees relying upon qualifying work experience gained outside the City and County of San Francisco must submit written verification of such experience as described above. Verification of experience may be waived if impossible to obtain. The applicant must submit a signed statement with the application explaining why verification cannot be obtained. Waiver requests will be considered on a case-by-case basis. Failure to submit the required verification or request for a waiver at the time of filing may result in rejection of the application.

SELECTION PROCEDURES:

The Certification Rule for this selection procedure will be Rule of the List. The names of all qualified candidates will be placed on the eligible list in rank order according to final score.
