Summary Statement

The incumbent in this position will oversee all community based operations funded by the division including case management, clinical services, staff development and family support services. Responsibilities include: collaborating with other DHSS divisions as well as other State and provider agencies, developing strategic short and long term goals to enhance service delivery based on best practices, implementing data driven assessment and decision making, developing policies and procedures to achieve positive outcomes and consistency of performance across the State.

Essential Functions

  • Directs home and community based services and supports for individuals with intellectual developmental disabilities, including the most complex, who are determined eligible for DDDS services; Develops and implements policies and procedures governing program operations.
     
  • Directs a unit of professionals who provide behavioral and nursing consultation to DDDS eligible consumers, along with transition coordination for individuals prior to graduating from school.
     
  • Provides oversight and direction to provider agencies who provide home and community based services.
     
  • Provides expert consultation and instruction in the development and implementation of individualized behavioral support plans.
     
  • Supervises case management staff in the development, implementation and monitoring of person-centered Plans of Care for each client enrolled in a DDDS Home or Community based federal program based on assessment of each individual’s strengths, preferences and support needs. 
     
  • Ensures that all community services staff providing direct services meet division training requirements.
     
  • Ensures the development and implementation of program and staff evaluations.
     
  • Participates on interdisciplinary team processes as necessary.
     
  • Represents DDDS on the Interagency Collaborative Team managed by the Department of Education.
     
  • Develops short and long range plans for the home and community based program and manages resources required to operate the program.
     
  • Establishes cooperative agreements with appropriate agencies and functions as a liaison with other agencies and programs.
     
  • Ensures that home and community based services are provided in compliance with appropriate state and federal standards and regulations.

Job Requirements

JOB REQUIREMENTS for Director DDDS Community Services
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
  1. Possession of a Masters degree or higher in a Behavioral or Social Science or related field.
  2. Three years experience in health or human services program administration such as overseeing and directing the development, implementation and evaluation of health or human services programs and services; planning and establishing short and long range program goals and objectives. Providing advice to other agency organizational units through consultation.
  3. Three years experience in strategic planning such as planning and mapping a path between the present and future usually for three to five years by determining key objectives, how to accomplish the key objectives, what strategies should be used, what activities would contribute to accomplishing the key objectives and developing performance measures to gauge and report progress or success.
  4. Three years experience in developing policies and procedures.
  5. Three years experience in operations management which includes planning, directing, coordinating, controlling and evaluating operations typically through subordinate supervisors.
