Summary Statement
A class incumbent coordinates treatment services for the mentally ill or emotionally disturbed adolescent and their families to readjust and achieve a social, economic, emotional and physical balance compatible with generally accepted norms.

Nature and Scope

A class incumbent performs specialized psychiatric social work under general supervision from a technical supervisor. Incumbent's prime objective is to facilitate the treatment and discharge planning of the adolescent to expedite release from the hospital and/or restrictive treatment settings. A significant aspect of this work is the design and implementation of aftercare treatment of the adolescent and their families. Incumbent provides consultative services to other state and private agencies.


Essential Functions

  • Participates in treatment planning and discharge planning in conjunction with the facility personnel as well as with members of other involved agencies to insure that the treatment results in the expeditious release of the adolescent patient.
     
  • Designs the aftercare treatment plans and referring clients to appropriate outpatient treatment facilities, community support systems, and/or residential treatment facilities and insures that the discharge recommendations and aftercare plans are carried out.
     
  • Interacts with clients' families interviewing members, observing family dynamics, assessing strengths or dysfunctions and adapts aftercare treatment plan to reflect their needs as well as those of the client.
     
  • Provides direct supportive services including home visits, individual and family counseling as well as family crisis intervention.
     
  • Monitors the outpatient treatment components to insure progress and resolve any problems that might arise.
     
  • Develops and maintains a "bank" of community resources to provide the necessary services for the adolescent and family.
     
  • Prepares monthly statistical, narrative and court reports, as required detailing caseload activities.
     
  • Prepares progress notes on each client. Participates in staff meetings.
     
  • Makes formal presentations before community groups, schools and/or professional organizations.

  • Knowledge of availability, role and functions of community resources.

  • Ability to interpret, explain and implement applicable rules and regulations.
  • Ability to deal constructively with aggressiveness, hostility and other emotional attitudes.
  • Ability to evaluate existing programs and make recommendations for improvement to best meet individual and family needs.
  • Ability to prepare narrative and statistical reports.
  • Ability to maintain effective working relationships with other service agencies, public and private, as well as State and Federal regulatory agencies.

Job Requirements

JOB REQUIREMENTS for Adolescent Treatment Services Coordinator
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:

  1. Possession of a Bachelors degree or higher in Behavioral or Social Science or related field.
  2. Six months experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individuals human service needs.
  3. Six months experience in making recommendations as part of a clients service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
  4. Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures.
  5. Six months experience in narrative report writing.
  6. Possession of a valid Driver's License (not suspended, revoked or cancelled, or disqualified from driving).
