Crossroads Case Manager (#CS9600)



TYPICAL DUTIES

The Case Manager explains program services and procedures to potential clients; conducts assessment and evaluation of the needs of new and continuing clients; coordinates the implementation of all aspects of the case plan; develops plan modifications as clients needs change or are resolved; provides counseling and support crisis intervention; act as liaison and facilitator between client and other agencies; recommends modifications in case management procedures; assists with general assessment of community needs; assist in developing programs to meet clients and community needs; maintain case records; evaluate individual and family case information; assesses problems; determines appropriate types and methods of treatment; initiates intensive long or short term treatment plans.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited College or University with a Bachelors of Arts Degree


AND

Experience: One year of experience providing counseling, case management and development of assistance programs for youth and families.

KNOWLEDGE

Basic principles of sociology and psychology; principles of human behavior; general case management principles and practices; social, economic, and cultural needs and issues facing groups within the community being served; community organizations and resources; and techniques of interviewing and counseling; basic principles of public relations.

ABILITY

Assess client needs and determine appropriate services; develop and implement case management plans; counsel and instruct clients and their families; gather and analyze data and prepare appropriate documentation; express self effectively orally and in writing; establish and maintain effective working relationships with a variety of clients, staff, and public agencies.
