Main Purpose of Job
The Contractual Community Health Outreach Worker I is the entry level of work providing health and disease prevention information to medically underserved populations in the community and assisting them in adopting healthy behaviors. The purpose of this position is to improve the health status of Caroline County residents by improving awareness of the medical assistance program and the affordable care act, also ensuring access to medical home, and increasing care providers' knowledge through health education and increased awareness of the Health Department and community resources. The incumbent will work closely with the Administrative Care Coordination Unit (ACCU) nurse. The successful candidate must be bilingual in Spanish/English to be able to assist the clients who speak Spanish which is 29% of the community' s population. This is a part-time contractual position working scheduled days, evenings and weekends doing outreach activities and working weekends and evenings to perform duties for shelters.
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
Education: Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience: None.
NOTE:
Experience providing assistance to individuals in a health care or social services setting may be substituted for the required education on a year-for-year basis.
SELECTIVE QUALIFICATIONS
The selected applicant must be bilingual in Spanish/English to be able to assist clients who speak Spanish.
LICENSES, REGISTRATIONS AND CERTIFICATIONS
Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator's license valid in the State of Maryland.
