Main Purpose of Job

The Contractual Community Health Nurse I is the intermediate level of direct nursing care to patients in various community settings and local health department clinics.  The purpose of this position is to protect and promote the health of Queen Anne's County's citizens by assisting the Adult Evaluation and Review Services (AERS) Program, the Medical Assistance Personal Care Program, and the Community First Choice Program by completing assessments and making appropriate referrals.  The incumbent will protect and promote the health of HIV/AIDS infected individuals and the larger community by conducting case management activities as a part of the Communicable Disease Program.  The employee will develop a plan of care using the nursing process which mobilizes personal and community resources to optimize the quality of life for individuals with physical, mental, and functional deficits, handling document assessments and plan of care according to AERS protocols.  This is a full-time contractual position which may require working evenings/weekends only as needed.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

Education:    Determined by the Maryland State Board of Nursing under the licensing requirements for Registered Nurse.

Experience:  None.

DESIRED OR PREFERRED QUALIFICATIONS

The successful candidate should have experience mobilizing personal and community resources to optimize the quality of life for individuals experiencing physical, mental and functional deficits.  It is preferred that the successful candidate should have experience with HIV/AIDS clients.  Experience in Emergency Medical Services would be a plus.

LICENSES, REGISTRATIONS AND CERTIFICATIONS

1.  Candidates for positions in this classification must possess a current license as a Registered Nurse from the Maryland State Board of Nursing, 4140 Patterson Avenue, Baltimore, Maryland 21215.

2.  Employees in this classification may be assigned duties which require the operation of motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator’s license valid in the State of Maryland.

SPECIAL REQUIREMENTS
