Main Purpose of Job

The Contractual Coordinator Special Programs, Health Services I is the entry level of work coordinating programs in the Department of Health and Mental Hygiene in a local health department, residential facility or the headquarters administration.  Employees conduct studies and implement assigned program activities in the areas of mental health, developmental disabilities, addiction and health services.  The Coordinator Special Programs, Health Services I position is the Developmental Disabilities (DD) Targeted Case Management (TCM)/Resource Coordinator provider who will act as the point person for all services offered to individuals served by DDA.  The incumbent will assist the individual in the preparation of the application and gathering of necessary documentation for DDA to make an eligibility determination.  The Resource Coordinator (RC) will be the responsible party for facilitation of the development of the Individual Plan (IP) and monitoring of the IP.  The employee will complete an assessment of most integrated setting and identification of barriers and a plan to overcome the barriers in the state residential center.

MINIMUM EDUCATION OR GENERAL REQUIREMENTS

Education: Possession of a bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.

NOTE:

A bachelor’s degree from an accredited college or university in another field plus one year of professional experience in health services may be substituted for the specific degree.
