Main Purpose of Job

This position functions as one of two of community placement specialists for the Department of Health and Mental Hygiene's Money Follows the Person (MFP) Demonstration Project. The main purpose of this position is to work with identified individuals in the State Residential Centers and nursing facilities to assist in their transition to community-based services through the Money Follows the Person Demonstration Grant and to monitor Peer Mentoring grant services.  This position must be able to work with a diverse transition team made up of the person desiring to move to the community supported by case managers, social workers, guardians, advocates, and staff from institutional and community settings.  The incumbent must be able to successfully overcome barriers on behalf of assigned individuals to community transitioning while adhering to departmental policies and procedures. The desired applicant should also have experience involving analysis of policies and procedures related to Medicaid waiver eligibility, accessing community based services, and working with individuals with disabilities and guardians/involved family members, case managers, social workers, and Medicaid waiver staff to achieve measurable goals and objectives directed by the person with a disability.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

Education: Possession of a bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.

Experience: Five years of professional experience in health services, two years of which must have been in professional work related to the support services and programs for developmentally disabled clients.  

Notes:

1. Applicants may substitute a master’s degree from an accredited college or university in a health or human service field for one year of the required general experience.

2.  A bachelor’s degree from an accredited college or university in another field plus one additional year of professional experience in health services may be substituted for the specific degree.

3. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The successful applicant should have training in Home and Community Based Services waivers, person-centered planning, principles of normalization, self-determination, and most integrated setting determinations, as well as experience in working with individuals with disabilities accessing an array of state resources and community supports.

BARGAINING UNIT STATUS

The incumbent in this position is a member of a covered bargaining unit and will be required to pay a bi-weekly service fee to the exclusive representative of the bargaining unit.
