Summary Statement

The incumbent will conduct developmental assessments, and provide service coordination and support to families of medically fragile infants/toddlers with identified developmental delay/disabilities.  Duties include: working as part of a multidisciplinary team, providing consultation to other team members, ensuring timely evaluation, developing Individualized Family Service Plan with families, securing appropriate early intervention services, facilitating the transition of eligible children for special preschool services through the local school district at age three, and managing a caseload.

  • Provides or evaluates direct nursing care.
     
  • Develops, implements and/or reviews nursing care plans.
     
  • Provides health care instruction and guidance to individuals, families, community groups and/or nursing home facilities in the management of their health care needs.
     
  • Maintains an effective working relationship with other members of the treatment team.
     
  • Prepares and submits records, reports, evaluations and forms. 
     
  • Participates in and make recommendations regarding quality assurance activities. 

Job Requirements

JOB REQUIREMENTS for Advanced Practice Nurse
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:

  1. Possession of a Delaware Advanced Practice Nurse license or temporary permit for a Delaware Advanced Practice Nurse license in Pediatrics, Public Health, Community Health, Maternal and Child Health or related specialty area.

Note:  Applicants qualifying based on a temporary license must have applied for licensure to practice as a Registered Nurse in Delaware or hold a valid Delaware Registered Nurse license and have graduated from a Master’s or certificate advanced practice nurse program.  The applicant must obtain the temporary Advanced Practice Nurse license permit prior to hire date and then possess full licensure credentials within a two year, three month period.   To obtain full licensure, candidates must complete 1000 clock hours of training and/or pass the certifying exam for their clinical nursing specialty, if applicable.
