The incumbent in this position will provide service coordination to families of medically fragile infants/toddlers with developmental delays or disabilities under the Child Development Program.. Duties include: provision of a multidisciplinary evaluation and assisting with the creation of an Individualized Family Service Plan if the child is eligible for services. In addition this individual will monitor service delivery; perform developmental evaluations and act as a consultant to family and staff.
JOB REQUIREMENTS for Advanced Practice Nurse
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
Note: Applicants qualifying based on a temporary license must have applied for licensure to practice as a Registered Nurse in Delaware or hold a valid Delaware Registered Nurse license and have graduated from a Master’s or certificate advanced practice nurse program. The applicant must obtain the temporary Advanced Practice Nurse license permit prior to hire date and then possess full licensure credentials within a two year, three month period. To obtain full licensure, candidates must complete 1000 clock hours of training and/or pass the certifying exam for their clinical nursing specialty, if applicable.
As a condition of hire you must possess a valid driver's license (not suspended, revoked or cancelled, or disqualified from driving).
